Welcome to the West Palm Beach VA Medical Center 

In addition to the forms listed on the main page of the VISN 8 Student Trainee Application Process website, please include the following items in your information packet:

-Two forms of picture identification, such as a driver's license and passport
-CPR Health Care Provider Card (American Heart Association is preferred)
-Copy of license if student is a licensed provider
-Current Resume (required only if your rotation is longer than 6 months)
-Trainee Qualification and Credentials Verification Letter (TQCVL) 
-Fingerprint Record Prep Sheet

If you are completing your fingerprint card at a location other than at the VA facility where you will be doing your clinical rotation, please include the following information in the upper left hand corner of the fingerprint form. The SOI/SON number for the West Palm Beach VAMC is as follows:

Your university coordinator will review your packet of information and can answer any questions you may have. Once the review is complete, please fax the documents to the West Palm Beach VA coordinator. Your faxed information is secure. The contact information for your VA coordinator is below.
