Student Trainee Application Process

We're so pleased you are accomplishing your training in the VA Sunshine Healthcare Network (VISN 8). You are on your way to becoming a part of the VA's proud tradition of providing the highest quality of care to those men and women who have so proudly served our nation.

This website is designed to help you successfully complete all required documents and training prior to beginning your rotation at the VA. The following steps are mandatory requirements for your student rotation with the VA Healthcare System.

IMPORTANT!! All required forms and training must be completed and faxed or emailed to the facility where you will be doing your rotation eight (8) weeks prior to the start of your rotation. The VA hospitals are working closely with your University Coordinators. If you have any questions, please contact your University Coordinator.

DIRECTIONS

STEP 1. Download all required forms. There are VA wide required forms for all hospitals and site specific forms for individual hospitals.  All VA wide and facility specific forms are required and must be faxed back to your training facility. At the bottom of this page you will find a list of the VA wide required forms. Click on the links to download the forms.

STEP 2. Click on the link below to the hospital where you will be working. Download any forms provided there as well. Save the forms to your electronic device.

STEP 3. Complete all required forms.  All forms must be filled out electronically; no handwritten applications will be accepted.  All forms are in portable document format (PDF) and can be completed with Adobe Acrobat Reader. To download Adobe Acrobat reader, Click Here

-Please fill out all forms completely. If something doesn't apply, please enter “N/A.”

-Make sure the documents are arranged in the same order as the checklist below.

-If you are getting your fingerprinting done at a VA facility other than the one you are doing your rotation at, you need an SOI/SON number specific to that VA facility. Please add the SOI/SON code to the upper right side of the fingerprint form. The SOI/SON numbers for each hospital are found on the individual VA hospital website links below.

-If you have any questions regarding the forms or required information, contact your college or University Coordinator for assistance. Please ensure that all information, including dates, is accurate. Incorrect entries may delay the start of your clinical rotation.

STEP 4. Once you have completed all the required forms, you will also need to complete mandatory VA TRAINING via the Talent Management System (TMS). To get started, follow the instructions on the TMS website .  To create your TMS account, you will be asked for specific information regarding the hospital where you will be performing your rotation. You can locate this information by clicking on the link to the hospital website below. Once you complete the training, you will receive a certificate. Print out this certificate to submit with your packet.

-The service coordinator listed on the rotation site will be your point of contact in TMS, not your preceptor.

-Use your school email to create the TMS account.

STEP 5. Please click on your assigned VA hospital below for additional forms specific to that hospital, for contact information to fax your forms, and for a map/directions:

STEP 6. Submit the packet of information to your college or university coordinator for review.  Once your college or University Coordinator has checked your forms for accuracy and completion, you will fax or e-mail them to the VA Facility Coordinator responsible to help you begin your rotation. (NOTE: DO NOT SUBMIT THE FINGERPRINT FORM NOW. YOU WILL HAND DELIVER IT AT A LATER DATE TO YOUR FINGERPRINTING LOCATION.)

STEP 7. Once you have received confirmation that the forms are received and accepted by VA, you will be asked to come to a VA facility to be fingerprinted and have a background check performed. For more information on fingerprinting, please contact your University Coordinator. The fingerprinting and background check process needs to be done six weeks prior to your start date. 

STEP 8. Once all your forms are completed and accepted, you will be ready for your VA rotation. You will receive additional information pertaining to when and where to report on your first day.

‘TO DO’ LIST - IMPORTANT DEADLINES:

1. Completed forms double checked by university coordinator and faxed to the VA hospital at least eight weeks prior to work start date.

2. Fingerprinting must be completed six weeks prior to work start date. Remember to bring two (2) forms of photo ID with you when you arrive for fingerprinting.
